CHIEF COMPLAINT
This is a case of a 80-year-old female, who was a farmer .
The patient presented to the hospital with chief complaints of

HISTORY OF PRESENTING ILLNESS
Detail history
Pt was 3rd born child among 6 kids to the parents
She had 2 brothers expired 10 years back and 1 sister expired 2 years
Married around age of 20
Husband was ( shepherd)
Expired 20 years ago
She has total 6 kids
1 son expired- chronic alcoholic
She was a farmer (20 years back)before stopped after her sons marriage taking care of theirs kids and cooking
Smokes sutta (cigars) since her childhood
Occasionally alcoholic
PAST HISTORY
PERSONAL HISTORY
- Appetite - Decreased since 10 days
- Micturition normal, no blood
- Increased bowel movements
- Sleep - inadequate
- Diet - Non-Veg & Mixed (Veg) sometimes
- No allergies
- Smokes Sutta (Cigar) since her childhood
- Occasionally alcoholic
- Marital status-Married
- Patient had married 50 yrs ago have 3 sons and 3 daughters
- One of her youngest son has expired two years back
- Both the daughter and son are married.
- Sons and daughter in law lives with her in the same home.
- Patient wakes up at am ,does her daily activities .........
- Takes J at around 7:30 and eats her by 8 am,.......
- Patient do her house hold work like washing her .........own cloths other small works and walks in home room to room daily ,talks with neighbours and have her lunch (roti & rice ) alternate days around by 2 pm , she sleeps afternoon till 5 pm and wake up around 5:30 takes Tea by 6 pm and have her dinner by 9 pm and sleeps by 10 pm.
